You need to turn in the medical release, athlete code of conduct, parent code of conduct, and physical in order to practice with the team.

Form	Description
Medical Release	Gives permission to provide medical care to athlete in the case of an emergency.
Athlete Code of Conduct	Agreement to follow school and team rules during the season.
Parent Code of Conduct	Parent agreement to act honorably during competitions.
Physical	Physicians consent to allow athlete to participate. Also informs coaches of any pertinent medical condition.
Anasazi Registration	Registration form for the Anasazi Camp. Send this to the address on the form. Also, email Coach Fogle to notify him that you will be attending.
Anasazi Insurance	Registration form for the Anasazi Camp. Send this to the address on the form. Also, email Coach Fogle to notify him that you will be attending.
